Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Per an incident report was not shared with the parent. Their signature in not on the report.”
“One child with a peanut and eggs allergy who needs an epi pen had a food allergy plan but it did not specify what medication or dosage is required if exposed. Also the epi pen was not on site as it expired and had not been replaced.”
“3 of 10 child files evaluated did not have a health statement within 12 months of admission as signed by the parent that they would provide.”
“Two caregivers without background checks submitted were present in two different classrooms at the operation. Note: The caregivers were sent home during the inspection. The director stated she submitted the background checks during the inspection.”
“The operation's franchise tax status is showing as forfeited on the comptroller's website today 1/23/25.”
“The last conducted health inspection was 11/3/23 which is over a year ago.”
“The last conducted health inspection was 11/20/23 which is over a year ago.”
“Information gathered in the investigation substantiated that supervision was not in compliance with minimum standards when a child was left behind in the classroom when the class transitioned to the playground.”
“Documentation of the health inspection was not available.”
