Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“At inspection, the operation did not have documentation for emergency fire drills for the month of June. This was corrected at inspection when the director completed the emergency drill form.”
“At inspection, two caregiver files did not contain the 2912, pre-employment affidavit form. This was corrected at inspection when the two caregivers, completed the 2912 form.”
“At inspection, one caregiver did not have a signed and notarized affidavit.”
“At inspection, a caregiver supervising the 18-23 month age group did not know the exact count of the kids she was supervising.”
“There was one child missing their full immunization shot record.”
“The facility did not have food menus, an employee list, or their recent 2936 form displayed.”
“A bucket in the outdoor play area contained about 2 inches of rainwater.”
“The children did not wash their hands before having a snack, nor were they instructed to do so.”
“One employee had not completed orientation at the center. This was corrected when the employee completed the orientation at the time of the inspection.”
