Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“4 out of 4 children's records reviewed did not have a health statement.”
“The outside area was not safe for children. There was a water hose lying in the yard which posed a tripping hazard, a shovel within the reach of children and trash in the yard.”
“The operation did not document the monthly test date for the fire extinguisher, smoke detector and carbon monoxide detector.”
“4 out of 4 children's records reviewed did not have current admission information.”
“The operation did not have crib documentation for the crib in the operation.”
“Three staff members did not have notarized affidavits on file.”
“The operation did not have a detailed emergency preparedness plan on file.”
