Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Three children's files lacked the immunization record for the specified age of the child.”
“Based on the preponderance of evidence gathered, sufficient evidence exists to determine deficiencies in maintaining medication administration records for a child, as the records provided were incomplete and lacked entries for several days during May, June, and July while the child was in care.”
“Based on the preponderance of evidence gathered from inspection and interviews, there was sufficient evidence to determine deficiencies related to the allegation. A child ingested the hearing aid battery.”
“Two children's records who met the age for hearing/vision screening did not have one in the file.”
“One child's record lack the health statement from a health care professional.”
“The seatbelt on one of the vans on the last row 4th seat was stuck on its non-use position.”
“The annual fire inspection had not been conducted. The last annual fire inspection was lasst conducted on 11/08/2023.”
