Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“All files reviewed were missing a form of photo identification. This was corrected at inspection when the operation had all employees provide a copy of their Photo ID for their files.”
“Multiple records reviewed were missing the child's physician or emergency care facilities information.”
“The operational policy was missing the required information for Outdoor/Physical Activity at the center.”
“None of the personnel files had the required information on hand during review.”
“The operation did not have proper documentation showing they had completed the required emergency drills.”
“No children's records reviewed had their physician information on file.”
“3 of the 4 records reviewed did not have the complete information for their emergency contact.”
“During review signed and dated infant feeding instruction were not available for review.”
“During the inspection an infant was observed sleeping in their crib while being swaddled.”
“During the inspection the infant cribs were observed and no form of label to identify the cribs were seen.”
