Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“According to the Texas Comptroller website, the business entity's Franchise Tax Account Status is Inactive/Forfeited. The operation is currently providing care to children with an inactive status.”
“An infant was anpping inthe crib with a bib around their neck.”
“All 16 infants not yet on table food had expired infant feeding instructions. expiration exceeded one month. One infant expired two months ago.”
“The first aid kit was missing scissors and thermometer.”
“I was not able to conduct an inspection because there was not someone available to open the front door. Also, the doorbell was not functioning because of Wi-Fi issues due to the weather. The doorbell alerts staff that someone is at the front door. Front door is kept locked and the only way you can enter the operation is by having someone open the door from the inside or by having a key to get inside. The operation's business phone is transferred to someone who is not at the operation. Upon calling that number, that individual wasn't successful in making contact with staff at the operation.”
“A mop bucket that had water was accessible to chiildren as it was located next door to where children were having their lunch.”
“Upon reviewing children's records, 6 out of 10 children's records were incomplete: operational policies were not addressed, name of physician and hospital/facility, signature, copy of immunization.”
