Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The operation started caring for school-age children without having school-age children on their permit and did not notify Child Care Regulation. This was corrected during the inspection when the director notified CCR that she has school-age children enrolled.”
“One of the outlets did not have a cover. This was corrected during the inspection when a cover was placed in the outlet.”
“Two of the infants' cribs were not labeled. This was corrected during the inspection when the cribs were labeled with the infants' names.”
“Out of four children's files reviewed, one child did not have an alternate emergency contact or release information completed.”
“Out of four staff records reviewed, one employee was missing their hire date, employment affidavits, proof of education, acknowledge of receipt of the operation's policies, and a statement that they attended orientation with an overview on abuse and neglect.”
“A slide less than five feet tall did not have loose-fill in the use zones. This was corrected during the inspection when the slide was removed from the playground.”
“The director's first aid and CPR training expired in January 2025.”
“The most recent inspection letter was not posted. This was corrected during the inspection when the most recent inspection letter was printed and posted.”
“One infant had infant feeding instructions that were last updated 12/2/24.”
“One child did not have updated immunizations.”
“The operation did not have a menu posted. This was corrected when a menu for the week was printed and posted.”
“The director and two caregivers did not have pre-employment affidavits, and their employment affidavits, start dates and background check letters were from the previous location where they worked before the new location started.”
“One child did not have a physician's health statement on file and needed an updated immunization record.”
“There was no current list of employees posted.”
“The parent handbook did not have information on vaccinations for employees.”
“The fire extinguisher, smoke detectors, and carbon monoxide detector were not tested in October. This was corrected during the inspection when the fire extinguisher, carbon monoxide detector, and smoke detectors were all tested.”
“The operation did not have a children's product certification form verifying that the product recall list had been reviewed.”
