A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“Based on investigation findings, the operation failed to contact the parents of a child who became ill while in care.”
“Based on investigation findings, proper diaper changing and health practices were not maintained when a child's diaper was not changed upon drop off to pick up. Additionally, another child's soiled clothes were not changed when the child had an accident on themselves at pick up.”
