Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Only 4 out of the 6 required leadership training hours were available for review.”
“One out of ten children's files did not have an immunization record available for review.”
“The director and two caregivers did not complete two hours of annual training of transportation safety.”
“Four fire extinguishers did not have documentation of current annual inspections being conducted. A fire extinguisher in a transportation van showed it was last inspected on January 2, 2024, a second van did not have a tag showing when it was last inspected, and a third van and kitchen had the fire inspection tags ripped in half, making it unable to determine when they were last inspected.”
“An infant room was over ratio by one infant. There were five infants in care between the ages of 3 months to 6 months with one caregiver. Note: This was corrected at inspection when a second caregiver was brought into the infant room.”
“During an investigation, it was determined that a caregiver used prohibited guidance by grabbing and pulling a child by their arms and hands due to behaviors exhibited by the child.”
“The teachers in classroom Toddler I stated they had 12 children, when it was verified, they had a total of 11 children.”
“Based upon information gathered and interviews conducted inadequate supervision was determined to have occurred when a child was found in a classroom alone.”
