A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“2 children were observed coming from and going to the bathroom unsupervised.”
“A caregiver was observed not washing hands after diapering a child. The caregiver used gloves which are for single use”
