Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The CPSC form was last signed April 2025.”
“Out of the four children's files reviewed, one was missing their physician's information, a health statement, and an official immunization record, and one needed an updated immunization record.”
“Out of the four staff files reviewed, two of the caregivers were missing a notarized employment affidavit form.”
“The last recorded fire drill and testing of the fire extinguisher, smoke detectors, and carbon monoxide detectors were in April. This was corrected during the inspection when a fire drill was conducted and the primary caregiver tested the fire extinguisher, smoke detectors, and carbon monoxide detectors.”
“Children under two years old were watching television with the older children. This was corrected during the inspection when the children under two were taken into a different room with another caregiver so they weren't watching television.”
“Out of four children's records reviewed, two required updated immunization records.”
“Out of three staff files reviewed, two were missing proof of education and pre-employment affidavits and all of them were missing copies of their photo IDs.”
“There was no children's product certification form available for review.”
“There was a caregiver present who still had a pending background check. This was corrected during the inspection when the caregiver was sent home.”
“The children's cots were not labeled with their names.”
“The parent handbook did not have plan for physical activity to occur indoors when weather is too bad to go outdoors, an emergency preparedness plan, procedures for parents to view minimum standards online and review inspection reports, and procedures for the home to do health checks.”
“There was a gap at the bottom of the fence large enough for a child to crawl under, and sharp edges of a cut chainlink fence where children could access them.”
