Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During the inspection walk through. a cover slip made of cloth was observed to be on the changing pad, in the infant room.”
“A five month old infant was observed sleeping on their side, on a pillow and wrapped in a blanket during nap time.”
“Based on the evidence obtained during the investigation, it was found that caregivers are pinching, spanking or both as forms of discipline.”
“Various classes had cleaning supplies within reach of children.”
“It was found that 3 out of 3 infants that are required to have Infant Feeding Instructions did not have updated forms that are signed and dated by the parents on file during the inspection. The forms on file at the operation were last signed and dated by the parents in June 2025.”
“It was found during the inspection the operation allowed a person to be present in a classroom and have access to children without receiving their eligiblity letter.”
“It was found during the inspection an employee changed a diaper on the children's play mat on the floor where other children were playing.”
“The following items are missing from the Operational Policies. 746.501(8)(9)(12)(13)(15)(16)(25)(26)(27)(28)(30).”
