Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“Observed breast milk bags that did not have the infants first name and last initial. This was corrected whent he infants name was written on the bags.”
“Observed a classroom using a wash cloth to sanitize the changing station, but was not changing the wash cloth between diaper changing.”
