Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“An older infant had their training cup next to them on their nap mat at naptime.”
“Based upon information gathered and interviews conducted, inadequate supervision was determined to have occurred when a child was found in the classroom unsupervised.”
