Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Two caregivers did not have 24 hours of annual training.”
“A child was separated from the group of children for disciplinary reasons and in the office for over an hour.”
“A child was made to sit in the office for 75 minutes as a punishment for not listening.”
“A child was separated from the group for over an hour.”
“There is enough evidence to determined that the caregiver who was supervising the child was not in close proximity of the child which resulted in the child being able to leave the facility. It was noted that another caregiver in a different classroom was watching the child and was able to immediately react to the child's escape.”
“There is sufficient evidence to determine that the caregiver in charge of the child did not adequately intervene to prevent the child from leaving the daycare facility.”
“Five children did not have the name, address, and telephone number of the child's physician or an emergency-care facility.”
“One child with a food allergy did not have an epinephrine auto injector as listed by the physician on the plan available for use in case of an emergency.”
“Six infants did not have feeding instructions reviewed, signed, or dated by a parent in the last 30 days.”
“One caregiver was present in the classroom with children without an eligible background check. The caregiver left during the inspection.”
“Two classrooms has garbage accessible to children. The staff removed the trash cans from the classrooms during inspection.”
“Five staff were missing annual hazardous materials training.”
“The operation did not have an admission record for a child in care.”
“Multiple infants were missing updated feeding schedules that had been signed by their parent.”
“The operation did not have their staff list, their last inspection form, or two of the required parent information posters posted.”
“Multiple caregivers were missing in-person CPR training that includes the use of hands-on training and practice with a manikin.”
“4 caregivers did not have the full 24 hours of annual training that are required.”
“Three classrooms were missing current posted activity plans for this week.”
“There were two pieces of equipment with slides on the playground that did not have loose fill surfacing underneath them. The equipment was removed during the inspection.”
“1 of 17 immunization records reviewed did not have the required doctor's record of immunizations. 1 of 17 immunization records reviewed did not have proof of the 2nd Hep A immunization.”
“Current fire inspection was not available for review.”
