Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Upon review of personnel files 3 staff were missing a copy of their photo ID indicating age requirements, 4 staff were mising proof of background check, and 3 staff were missing affidavit 2985.”
“A play yard in the toddler room was observed to have an extra padded mattress that was not provided by the manufacturer. This was corrected at inspection when the padded mattress was removed from the play yard.”
“It was determined throughout the course of the HHS investigation that child in care were not supervised by the caregiver when the caregiver was assisting another child in care in the adjacent restroom. A young child in care received a minor injury as a result of inadequate supervision.”
“It was determined throughout the course of the HHS investigation that a child in care was released to family members not listed on the admission paperwork.”
“It was determined throughout the course of the HHS investigation that a parent was not notified of less serious injuries at pick-up.”
