Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Three smoke detectors in the building were observed to be beeping and in need of batteries during the inspection.”
“There was one caregiver observed in a classroom with 8 infants ages 10 months to 19 months old. An additional caregiver was added to the classroom during the inspection.”
“There were multiple maintenance deficiencies observed on the two outdoor areas of the operation. There were active ant piles observed, bags of mulch, bird waste, water hoses accessible, a broken gate, broken toy storage and miscellaneous broken items observed.”
“Observed piles of leaves, with large sticks and a small amount of trash in the leaves on the back playground. I spoke to the director about getting the playground cleaned up for use.”
“Director failed to notify CCR within 2 days of change in location of services.”
“The fire inspection was past due.”
“One child who receives a breathing treatment daily did not have a medication authorized signed and available for review at the time of inspection.”
“There were several cribs that had a crib mattress and a foam mat. This was corrected at inspection when the foam mat was removed. Several crib mattresses were torn. This was corrected at inspection when the mattresses were replaced with new ones.”
“The crib mattresses covers were not snug.”
“The back playground is in need of a deep clean. Sand box has green sand and grass growing in the sand, bird feces was observed on some of the materials.”
“Two out of six children files reviewed were not current on immunizations or did not have a record for review at the this inspection.”
“Two of out of the five records reviewed did not have current CPR and First Aid training.”
“There were two fire extinguishers that had expired as of March 2022.”
“The air conditioning units were accessible to children and did not have proper safe guards to prevent access.”
“Out of five caregiver training files reviewed two caregivers did not have current training certificates available for review at the time of inspection.”
“Out of five staff files reviewed, three caregivers did not have current required training certificates available at the time of inspection.”
“The operation did not have records available for review to determine when the child was given medication.”
