Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The operation failed to provide a designated group of children with their required two daily opportunities for outdoor play”
“The operation failed to securely mount the electric fan to the wall, creating an immediate physical hazard within the infant care area”
“The swing sets feature an improper layout where their operational use zones overlap directly with the use zones of the slide structures.”
“The operation failed to maintain the cleanliness of its rugs, floors, and walls.”
“The operation failed to maintain formal, written documentation proving that a standard health and sanitation inspection is not required the operation.”
“The operation failed to include all required information within its operational policy regarding the promotion of indoor and outdoor physical activity.”
“The outdoor playground area contains several pieces of stationary equipment that lack the mandatory six-foot use zone around their perimeters.”
“During the inspection medication for one child in care was found to be expired. The injection has been expired since Janurary 18, 2025.”
“Diaper changing equipment was torn. The director replaced the equipment with a new mat.”
