A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“Observed a child under 12 months old sleeping in a Play yard with a pillow and toys inside.”
“The conditions imposed during the probation period were not completed.”
“There was no fire drill logged for the month of September.”
