Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Caregiver did not have a current first aid certification for review.”
“Two portable fans were stored on top of two shelves in the PreK classroom. Note: This was corrected at inspection when fans were removed from the shelves.”
“A mop bucket with water was accessible to children in an adjacent classroom through an opened door. Note: This was corrected at inspection when mop bucket was removed.”
“Caregiver did not have a current pediatric cardiopulmonary resuscitation (CPR) certification for review.”
“Caregiver was providing care to children in the infant classroom without another person supervising the caregiver. Note: This was corrected at inspection when another caregiver was placed in the classroom.”
“Five of eight staff files reviewed during inspection did not include the following annual trainings for the last full year of employment: Administering medication, handling and disposing of hazardous materials, preventing and responding to emergencies due to food or an allergic reaction.”
“There were two infants that did not have updated feeding schedules at time of inspection.”
“Two staff did not have an affidavit on file at time of review.”
“2 infants were seen sleeping in swings. This was corrected when the infants were moved to thier cribs.”
“5 of the 10 files audtited are missing all of the required trainings for health and safety topics in section e of annual training hours for caregivers.”
“the operation does not have the proper documentation of the gas inspection available for review during a CCR inspection.”

“Six out of ten staff files evaluated were missing CPR and First aid training. Note: Operation had a schedule training for all staff on the following day.”
“A staff member's purse in the five-year-old classroom was sitting on the children's low open shelve accessible to the children in care. Note: This was corrected at the inspection.”
“A caregiver was found prohibiting a child to play with toys or in centers due to the child's daily behavior.”
“A caregiver had documented inappropriate discipline measures as a child colored the floor. The caregiver made a report stating that the child will clean the floor first thing the next morning. It was documented that the child was put to clean floors at arrival time.”
