Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During the course of the investigation, it was determined that a caregiver did not demonstrate good judgement when they provided first aid treatment of hydrogen peroxide to a child's staph infection without a doctor's order or parent's authorization.”
“A current list of employees was not posted. This was corrected at inspection when a current employee list was posted.”
“The emergency preparedness plan was incomplete.”
“The operational policies are lacking several required topics such as: a statement that medication is not administered, a complete section on indoor/outdoor activities that includes the promotion of physical activity, duration of physical activity, type of physical activity, recommended clothing/footwear for physical activity, criteria used to determine extreme weather conditions, and a plan to ensure physical activity will still take place when weather does not allow for outdoor time. Also missing were: parent rights, information on vaccine preventable diseases for employees, and a statement on a gang-free zone.”
“The permit renewal notice was not posted. This was corrected at inspection when the permit renewal notice was posted.”
“The most recent licensing inspection or investigation form was not posted. This was corrected at inspection when the most recent inspection forms were posted.”
“School age children were allowed to have screen time during their naptime or rest period. This was corrected at inspection when it was discussed with the operation and it was determined that this standard will be discussed with all caregivers. A screen time handout was also provided and will be reviewed with all staff.”
“Wall sits were utilized as a form of discipline for seven school-age children.”
“Two structures in the playground with a platform above 20 inches do not have a 6 foot use zone.”
“Three out of six staff members files reviewed did not have a notarized affidavit of employement.”
