Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on the information obtained during the investigation, surfacing was not maintained to the required depth of 6 inches.”
“5 staff files were evaluated and two did not have affidavit on file. NOTE: CORRECTED AT INSPECTION as affidavit was completed at the time of inspection.”
“In one classroom, inspector observed three cans of electronic cleaner (dust and lint remover) stored in a cabinet that was accessible to children. NOTE: This was corrected at the time of inspection when caregiver removed and placed the items out of reach of children.”
“The two caregivers in the nap room did not know how many children they were responsible for.”
“A teacher used inappropriate discipline with a child when pulling their arm.”
“September's fire drill was not conducted.”
“Based on the information provided during the course of the investigation, staff restrained children during naptime while in care. Staff have also restrained children in other situations while in care.”
“Staff at the operation also used poor judgment when they disregarded policies put in place approximately one year ago in regards to handling children and not putting any part of their bodies on them. One of the staff involved did sign that they acknowledged the new policies.”
“Based on the information provided during the course of the investigation, the director failed to properly supervise staff during hours of operation. A similar incident had occurred approximately one year ago and Inspector discussed the situation with the director at that time as well.”
“Based on the information gathered, two children were able to gain access outside the daycare gate and walk .4 miles to a nearby neighborhood. The children were missing for approximately ten minutes, until found safe by a daycare parent.”
“The director was unable to provide trainings for one staff member who did not have the required trainings for this section that are needed for pre-service at the time of inspection on 10/13/2021.”
