Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“There is a violation of this standard, as non-positive methods of redirection and discipline were observed to handle challenging behaviors.”
“Children toothbrushes were touching each other during storage. Also, were accessible to children.”
“The operation has not maintained the annual fire inspection as required. The last fire inspection was conducted on 11/13/2024.”
“There is a violation to this standard, as a child was left unsupervised in the playground area for 3 minutes.”
“Child Care Regulation was not notified of a change to the director position within the required 5 days.”
“A door that did not latch into place when closed remained unrepaired and contributed to a supervision incident.”
“A child exited the property and entered the street unsupervised.”
“Two children's admission paperwork did not include a name, address, and telephone number of another responsible individual (friend or relative) who should be contacted in an emergency when the parent cannot be reached.”
“One caregiver hired after 9/1/23 did not have a Pre-Employment Affidavit.”
“Three children did not have current immunization records.”
“Two staff personnel records did not have documentation showing how the employee meets the minimum education requirements, specifically, a GED/High School diploma or transcript.”
“Two children's files did not have the name, address, and telephone number of the child s physician or an emergency-care facility.”
“One caregiver did not have a complete notarized Licensing Affidavit for Applicants for Employment form.”
“One employee was missing documentation showing orientation was completed.”
“The operation permitted a person to be in direct care of the children before receiving an eligiblity letter. NOTE: The background check was submitted during the inspection and the caregiver was sent home.”
“A video tape was reviewed of 9/29/2021 incident. There were fourteen 3yo-5yo observed in care. A child was observed left in the classroom bathroom unattended and in the classroom for 2 minutes. The caregivers had taken thirteen children outside to the playground without the child while in the bathroom.”
