Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The provider has not inputted the number of employees that have left for the year of 2025.”
“During the monitoring inspection, it was observed that one infant bottle was in the refridgerator without a label. NOTE: This was corrected at inspection when the caregiver labeled the bottle.”
“During the monitoring inspection, it was observed that an area of the unitary surfacing had a hole as well as a gap between tiles.”
“One outlet was uncovered and accessable to kids. NOTE: This was corrected at inspection when an outlet cover was added.”
“An infant was asleep in a baby swing. It was stated the baby had been asleep for about 10 minutes. NOTE: This was corrected when the baby was removed from the swing and put into the crib.”
“Two directors did not have transportation training. A link to a transportation training will be provided.”
“One classroom did not know the number of children present in the classroom.”
“The operation was informed of the situation and did not report it to licensing.”
“A caregiver was seen grabbing the neck and back of a child in a video.”
“The operation did not share an incident report with the parent.”
“On bus 47, the first aid kit was missing the thermometer. Note: the thermometer was place on the bus during this inspection.”
“Seven of ten children's files reviewed did not have the date of admission. Note: the dates were added during the inspection.”
