Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One child's record was lacking a preschool health statement. The child has been in care since August 2024.”
“One child's record was lacking a current immunization record.”
“Two caregivers failed to provide the level of appropriate infant supervision necessary to prevent an infant from being trapped in the safety belt of a highchair, thus resulting in red marks being left on the infant?s neck. The injury did not require medical attention. The caregivers responsible were terminated as a result of the incident.”
“Camera footage revealed that caregivers were using harsh/profane/inappropriate language while handling the infants in care. These caregivers were terminated for their actions.”
“Camera footage revealed infant caregivers were not consistently washing hands after diaper changes, and a caregiver admitted to inconsistent hand-washing practices as well.”
“Camera footage revealed a caregiver grabbing/pulling several mobile infants up by their arms (e.g., via one arm) and carrying them across the room. This caregiver was terminated for her actions.”
“An infant wasn?t receiving prompt diapering as evidenced by a diaper rash/redness being discovered by his parent at pick-up along with a full, unchanged diaper.”
“Camera footage revealed a caregiver kicking/nudging an infant with her foot as the infant walked to the caregiver?s recliner, causing the infant to fall down. The infant was uninjured. The caregiver responsible was terminated for her actions.”
“Camera footage revealed that infant changing tables were not consistently sanitized via the 4-step process after each use, and a caregiver admitted during interview with DFPS staff that this was not consistently done as well.”
