Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“An individual was present at the operation and had access to children in care without having received a notice of eligibility by the background check unit.”
“Documentation was not available for review showing that staff are checking and inspecting equipment.”
“It was determined that one caregiver exceeded the amount of children one caregiver can be in ratio with.”
“I observed a child in care in a hallway unsupervised. The staff member responsible for the child's care was in a bathroom with other children. I also observed a staff member on her personal cell phone, while she was being counted in ratio.”
“Operation added outdoor space and children were already using the space prior to the space being inspected and measured.”
“Incident/Illness Report was not shared or signed by parent within the 48 hour requirement.”
“During naptime, a child was lying on a cot near a table. There were chairs sitting on top of the table. The child was messing with one of the chairs and one ended up falling on the child. The child sustained a cut slightly beneath his left eyebrow. The child required stitches.”
“Upon reviewing children's records, one staff did not have a notarized affidavit.”
“A substitute was present and was being counted in ratio. Substitute did not have a background check. This was corrected at inspection when the individual left the operation.”
“Operation did not conduct a fire drill for the months of August 2021 or October 2021.”
“Operation rotates menus. Menu's were not dated.”
“Four groups of children that were observed were out of ratio. Two groups were over by 1 child and two other groups were over by 2 childlren. Each group only had one caregiver counted in ratio.”
“A child was left in a transportation bus without supervision.”
“All children were not accounted for upon arriving at the operation.”
