Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“This standard was evaluated as part of an investigation. It was determined that a caregiver thumped a child on the ear and hit a child on the arm.”
“This standard was evaluated as part of an investigation. It was determined that a caregiver disciplined her own child using prohibited methods while the child was in care at the operation.”
“The annual fire inspection was unavailable for review.”
“This standard was evaluated as part of an investigation and found to be deficient. It was determined that two toddlers were able to exit out of the front entrance of the operation without caregivers being aware for approximately three minutes.”
“Children were observed slipping on pea gravel surfacing while climbing the stairs of the playground equipment structure. A large amount of pea gravel was on the stairs of the slide.”
“This standard was evaluated as part of an investigation and found to be deficient. Caregivers are unable to control the school age classroom at times. Additional caregivers or volunteers are needed to assist in the supervision and redirection of the children based on behavioral challenges. There was insufficient evidence to prove a child required medical attention due to a head injury while in care at the operation.”
“This standard was evaluated as part of an investigation and found to be deficient. A child injury occurred as a result of a caregiver mopping the classroom with children in care causing a slipping hazard.”
“This standard was evaluated as part of an investigation and found to be deficient. A caregiver was performing janitorial duties with children in care.”
“This standard was evaluated as part of an investigation and found to be deficient. The operation did not report a child injury requiring medical treatment by a Health Care Professional upon awareness.”
“This standard was evaluated as part of an investigation and found to be deficient. Adequate supervision was not provided for the children in care.”
“This standard was evaluated, and found to be deficient. The caregiver did not have appropriate visual and/or auditory awareness, and was not in physical proximity to intervene, and ensure safety of the children in care.”
“This standard was evaluated, and found to be deficient. Caregivers did not use good judgement in serving a snack food to an infant child, without parent permission, which caused a digestive reaction for the child.”
“One child did not have the required Food Allergy Plan.”
“All caregivers need the required Abuse/Neglect Training.”
“This standard was evaluated, and found to be deficient. The operation failed to report timely.”
