Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A caregiver referred to children with a derogatory term.”
“The operation sprayed over-the-counter bug spray without being a non-commercial applicator.”
“Based on a video observed for the investigation, the television was observed to be on during naptime.”
“A caregiver grabbed a child and pulled child to ground.”
“The caregiver did not immediately have the children wash their hands when they returned from playing in the playground. Corrected at inspection: The children's hands were washed during the inspection.”
“One out of four staff training records reviewed did not have training records in the required training topics of food allergies, infection control, and emergency preparedness.”
“The director was lacking training in the required training topics of food allergies, infection control, and emergency preparedness.”
“The door in the toddler room that leads to a hallway that goes to the kitchen was not closing. A mushroom playstructure in the playground had broken items that had sharp edges. They also began removing a tree from the playground, but didn't finish. It left a hole that created a tripping hazard in the playground.”
“There was a hole in the sheet rock accessible to the children in the three and four year old classroom bathroom.”
“At the time of inspection there was a mop bucket filled with water accessible to the children in the three year to eight year old class. This was corrected when the caregiver removed the mop bucket and placed it out of the reach of the children.”
“One out of nine children's file was lacking a health statement.”
“The permit renewal letter posted was expired in April 2022.”
“One of the nine children's files reviewed did not have the doctor information listed for the health statement.”
“A caregiver threatened a child by stating, "Chuckie was going to get them" if they did not go to sleep during naptime.”
“A 3 year old child was enrolled and attended the center for a week before being unenrolled because they were not potty trainined. According to the center's own operational policies, they would not enroll any child in their Pre-K or After School program if they were not potty trained. Corrected at inspection: The policy will be kept as is and they will no longer be accepting any child in the Pre-K or Afterschool programs that are not potty trained.”
