Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The use zone between structures on the playground was less than the required four feet for each structure.”
“Two out of ten staff are missing A Pre-Employment Affidavit for Applicants for Employment at Certain Child Care Operations (Form 2912).”
“Ten of ten children's records did not have addresses for emergency contacts.”
“Two classrooms with diaper changing tables did not have access to a handwashing sink.”
“The rooms did not have posted dated written activity plans.”
“Six foot use zones were not maintained around playground equipment. This was corrected when the director instructed caregivers to reorganize the playground equipment and remove toys from fall zones.”
“A child was sleeping positioned directly in the exit path of the classroom doorway. This was corrected when the child was removed from the exit.”
“Room 3 was too dark to visibly see the children in the classroom during naptime. This was corrected when the blanket was partially removed from the window to provide light.”
“The operation did not have a current fire inspection report. Last on file was 07/12/2021.”
“One caregiver in Room 6 was using her cell phone at the table while supervising children who were napping. This was corrected when the caregiver put away her cell phone.”
