Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During a walk through of the operation, it was observed two caregivers supervising a group of children without an active background status. This was brought to the person in charge attention and the caregivers left the operation while licensing was present.”
“During a review of the staff records, it was observed one staff member with an expired CPR/ first aid.”
“During the insepction the water hose was observed out and not put up after sprinkler play.”
“During an inspection of the playground, it was observed an excessive amount of "sticker burrs" all around the playground and along the walking paths.”
“This standard was reviewed in relation to the self-report allegation and found to be deficient. The operation did not notify licensing in the appropriate time frame.”
“This standard was evaluated at the monitoring inspection and the latest fire inspection report did not have a date of inspection and current inspection is expired dated 1/9/2019.”
“4 out of 10 training records did not meet the required 24 hours of annual training. One caregiver needs 22 hours, two caregivers need 19 hours and one caregiver needs 12 hours.”
“The operation purchased a new van and the van was not equipped with an electronic alarm.”
“The operational policy did not include information regarding indoor and outdoor physical activity.”
“The loose-fill under the climbing equipment does not measure and grass is growing in the mulched area.”
