Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“There was not an infant daily schedule for me to verify if oppertunities for outside play and large and small motor”
“Of the 6 children files reviewed, one needed updated immunizations.”
“Of the 6 children's files reviewed, three needed complete address and telephone number for the parent's prefered doctor or hospital.”
“We were unable to find infant written feeding instructions for the infants under 12 months.”
“A vape pen fell from the pocket of a caregiver. It rolled on the floor, and a 20-month-old toddler picked up the vape pen, and held it out to her caregiver to take back.”
“Five out of the five staff trainings records that were reviewed were missing required training hours.”
“None of the classrooms had a posted activity plan.”
“The loose-fill in the outdoor play area did not meet the 6 inch requirement.”
“One caregiver's CPR and First Aid training expired in June 2024.”
“The director was missing required annual training hours.”
“A caregiver was present who did not have a current eligible background check. The caregiver left during the inspection.”
“The emergency practice log was not available for review.”
“Three caregiver's personnel records were missing their proof of education.”
“One child's file did not have a health statement.”
“The playground maintenance checklist was not available for review.”
“It was determined during the course of this investigation that a caregiver did not check the label on a bottle, resulting in an infant receiving the wrong bottle.”
“During the walk through of Classroom 3 two broken outlet plates were observed.”
“The operation did not have a current fire inspection. The last fire inspection was completed on 8/5/2020.”
