Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on the evidence obtained, a child left the classroom and was unsupervised in the hallway. The caregivers in the classroom were not aware that the child had exited the room.”
“Based on the evidence obtained, a caregiver failed to conduct an accurate count of the children they were responsible for when transitioning from the playground to the classroom. This resulted in one child being left on the playground for 10 minutes. Other caregivers and children remained on the playground but none of the caregivers on the playground were aware that the child was left nor that they would be responsible for ensuring responsibility for the child. The caregivers that were aware they were responsible for the child were not within physical proximity when they entered the building and lost both auditory and visual awareness of the child during those 10 minutes.”
“Based on the evidence obtained during the investigation, caregivers did not follow food allergy protocols, and a child was served a food item on their food allergy list.”
“Based on the evidence obtained during the investigation, a child was served a food item on their food allergy list.”
“Based on the evidence obtained during the investigation, a child was served food that was listed on their identified foods as a known allergy to avoid.”
“The most recent gas leak test was not available for review.”
“There were 7 infant feeding instructions reviewed that were not updated in the last 30 days by the parent.”
“Based on the evidence obtained, the parent was not immediately notified of a situation that placed their child at risk.”
“Based on the evidence obtained during the investigation, a child was left unsupervised in an enclosed hallway for three minutes before being returned to their classroom.”
“Based on the evidence obtained during the investigation, two prior incidents have occurred where a child with a diagnosed food allergy were served food listed on their food allergy plan and resulted in an allergic reaction.”
