Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Out of six children's records reviewed, one child was missing a health statement and two needed updated vaccination records.”
“One child had a food allergy emergency plan that was not signed by the parent.”
“There was a slide and a piece of rocking equipment on the playground that had tags indicating that the equipment was for residential use only. This was corrected during the inspection when the equipment was removed from the playground.”
“During the investigation, it was found that two children were able to leave their classroom unsupervised. One child was outside of the classroom for approximately two minutes, and the other was outside of the classroom for approximately one minute.”
“During the investigation, it was found that parents were not notified immediately after their children left the classroom unsupervised.”
“During the investigation, there was sufficient evidence found to determine that the rugs and garbage can in Classroom D were dirty and had food on them.”
“During the investigation, there was sufficient evidence found to determine that a child who was under two years old was allowed to play on a play structure designed for children ages two to five years old.”
“During the investigation it was found that a caregiver grabbed a child's arm.”
“During the investigation it was found that one caregiver used verbal threats of corpral punishment towards a child.”
“Certificates of completion for caregivers annual training hours were not available for review.”
“There was not an emergency food allergy plan available for one child with a diagnosed peanut allergy.”
“It was found that force was used to keep a child on a mat during nap time.”
