Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“It was observed that during nap time, one caregiver was seated between two classrooms of napping children (one classroom with 18 children ages 3-5 and another classroom with 15 children ages 4-7 years old). Another caregiver arrived during the inspection correcting the nap time ratio.”
“During a DFPS investigation, it was found that the operation staff interfered with the investigation by not allowing a parent to speak with DFPS staff while at the daycare. It was also found that the daycare staff told untrue accounts about when the child left the operation and the day it occurred.”
“There was sufficient evidence found during the DFPS investigation to show that the operation did not report a situation placing a child at risk to Child Care Regulation within the required timeframe.”
“There was sufficient evidence found during the DFPS investigation to show that the operation did not notify parents of a situation placing their child at risk in a timely manner.”
“There was sufficient evidence found during the DFPS investigation to show that adequate supervision was not provided when a child was able to exit the building to the parking lot, towards a roadway.”
“There was sufficient evidence found during the investigation to show that infant feeding instructions were not followed for one infant.”
“During a DFPS investigation it was found that two children did not have admission forms for review, and the operation only had the parents' phone number.”
“A caregiver stated that there were 32 children in the classroom. There were 41 children present.”
“There were 41 children ages 2 years to 7 years old present in a classroom with one caregiver. The class was split during the inspection when another caregiver arrived at the operation.”
“A caregiver stated that there were 15 children in the classroom. There were 13 children present.”
“Infants had a tablet with cartoon folded over the cabinet for viewing.”
“4 infants under the age of 12 months were in their cribs laying on boppy pillows or had a blanket covering them.”
“Infant feeding instructions were observed not to have been updated every 30 days.”
“There were no written feeding instructions available for review.”
“During an investigation, it was found a child was left unsupervised on the playground.”
“During an investigation, it was determined that a child was released to someone without checking their identification or verifying they were on the child's authorized pick-up list.”
“During the inspection, ten infants were observed in one classroom with one caregiver. Also, twenty-four two-year-old children were observed in one classroom with one caregiver.”
“The designated person in charge did not have access to the operation's records. The person in charge stated children's records and personnel records are kept in a locked cabinent and she does not have a key to unlock it.”
“A towel was observed in a crib with an infant younger than 12 months. The caregiver removed the towel from the crib.”
“A 5 month old infant was observed drinking a bottle that was propped with a towel. The towel was removed and the caregiver carried the infant to feed them.”
