Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Children in care have been subjected to having their hands tightly squeezed as a form of discipline by a caregiver.”
“A child left the classroom and walked to lobby area without any adult supervision. Children walked out the classroom and walked over to the director's office without permission.”
“The operations did not have a current fire inspection.”
“Eight children's files were reviewed. Two children's files did not have health statements.”
“Based on the information obtained through the DFPS Investigation, there is sufficient evidence to show that a caregiver grabbed and pulled a child to move them.”
“Based on the information obtained through the DFPS Investigation, there is sufficient evidence to show that a caregiver threatened to put a child into a cubby.”
“Based on the information obtained through the DFPS Investigation, there is sufficient evidence to show that candy has been provided to a child to keep them calm while in time out.”
“Based on the information obtained through the DFPS Investigation, there is evidence to show that a caregiver did not demonstrate self-control or good judgement when they put a child?s shoes in the trash can.”
“Four children did not have an emergency contact address on their enrollment form.”
“Three out of ten staff records reviewed complete CPR training online (National CPR).”
“During the walk through one trainee was counted in ratio with a pending background check. Note: Trainee left the building.”
“The parent was not notified of the child's ant bite at pick up.”
“Last fire inspection conducted was dated for 2/25/2021. Current fire inspection could not be observed.”
“A caregiver was observed in the Toddler classroom without an active background. Corrected at inspection: caregiver was asked to leave.”
