Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The fire inspection expired on 9/22/2026.”
“2 infants did not have current feeding instructions on file.”
“A 5 month old was asleep in the crib with a blanket. Note: The blanket was removed during the inspection.”
“There has been no documented monthly playground checklist. A Daily buildings and grounds checklist was being completed.”
“3 employees who transport did not present the transportation safety training.”
“2 out of 8 staff files reviewed did not the required 24 hours of annual training.”
“Based on information provided during the investigation inspection and interviews there is a sufficient amount of evidence to suggest that the counselors did not properly supervise children in their care.”
“4 out of 9 children records reviewed did not have a physicians health statement.”
“The Safety Alarm on bus was turned off.”
“The operation did not have an open-door policy at the investigation time.”
“Two caregivers' files did not have a current pediatric first aid.”
“Two caregivers' files did not have a current pediatric CPR.”
“Two children's records required the heatlhsttatemne from a health care professional. One child's record required the health statement from the parent.”
