Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A caregiver did not wash a child's hands after changing their diaper.”
“This standard was evaluated in regard to an investigation and a violation was found. A caregiver was pulling a child by the arm and another child by the legs to move them to a different space on the carpet instead of using positive redirection.”
“A vendor was present in the the classroom with children that did not have an eligible background.”
“The operation did not have a current fire inspection.”
“This standard was evaluated in regard to an investigation and a violation was found. A caregiver grabbed a child and place them on their nap mat in an abrupt manner instead of using positive words to redirect the child's behavior.”
“This standard was evaluated in regard to a DFPS investigation and a violation was found. A 3-year-old child was left unsupervised on the playground for approximately 4 minutes.”
“Medication was stored at the operation that was past its expiration date.”
“3 infant feeding instrucion forms have not been updated in the past 30 days.”
“2 out of 10 children's records reviewed did not have current immunizations.”
“The operation did not have a current fire inspection.”
“Medication for a child was not in the original container.”
“2 out of 4 infant feeding instructions reviewed were not current.”
“2 out of 10 children's immunization records reviewed were not current.”
“The operation did not have a current fire inspection report available to review.”
“The electronic child safety alarm on the bus was not working or being maintained by the operation.”
“The director did not have current transportation training.”
“Two children's files were observed and did not contain current immunization records.”
“The loose fill on four different areas of the playground were observed without the proper depth of wood chips. Two areas on the school-age playground were observed with 4.5 inches of wood chips. The equipment requires 9 inches of loose fill. Two areas on the preschool playgrounds were observed with 1.5 to 3 inches of wood chips. The equipment observed requires 6 inches of loose fill.”
