Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Four out of four administration forms have not been updated and signed off by parents since 2023.”
“Four out of four children's files did not have an up-to-date health statement available to review.”
“The provider did not have an affidavit filled out at time of review.”
“Two long rifles were observed to be on display in an upstairs bedroom.”
“The primary caregiver does not have training in the administration of medication.”
“The operational policies do not contain a statement promoting physical activities.”
“The operational policies do not contain a notice to parent about the dog on the premises nor the turtle and bearded dragon in the classroom.”
“Two of four children's files do not contain a signed health statement.”
