A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“The operation was missing ID's and proof of education for two caregivers. They were missing a notarized affidavit for one caregiver and the pre-employment affidavit for another caregiver. One caregiver did not have her annual trainings available during inspection.”
