How to read violations

Nearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

Paperwork — records, postings, forms Standard — supervision, ratio, training Health & safety — direct risk to a child

Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

746.1601 — Child/ Caregiver Ratio - 13 or More Children

“At the time I arrived at the inspection, both infant rooms were out of ratio. The young infant room had 6 children and one caregiver. A second operation staff arrived, and went into the room, putting that room in compliance. The second room out of ratio was the 12-18 month old room. There were 8 children in the room. The ratio for this age group with one caregiver is 5 children.”
