A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“The operation failed to submit the number of employees that left the operation in 2024 via provider portal.”
“During the walkthrough the caregiver in the nap room did not know the number of children the caregiver was resposblie for. Caregiver had to count and stated 20 children but there was 19.”
