Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Two out of six files did not have the emergency care authorization filled out in their file at review.”
“The operation did not have hire dates for two of the staff files at review.”
“The operation did not have copies of photo ID's for three files at review. *This was corrected at inspection*”
“The back gate to the playground will not close. *This is due to the ground shifting*”
“Operation did not have feeding instructions for infants not ready for table food.”
“The operation did not have a playground checklist for the past month at review.”
“Two out of six records reviewed did not have current immunizations at review.”
“A hose was left out on the playground not in use where children were accessible to it which could have caused a tripping hazard. This was corrected when the director wrapped the hose back up. There was a rain gutter located on the playground that was missing a cover around the edge of it that is sharp to the touch. This was corrected when the director covered the edge of the gutter with a pool noodle.”
“The parent rights information was not available for review in the parent handbook.”
“The promotion of physical activity was not available for review.”
“There were no sanitary gauze pads in the first aid kit. This was corrected when the missing items were added to the first aid kit during inspection.”
