Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One of the ten children's files reviewed did not have another listed adult to notify in the event of an emergency.”
“One of the ten files reviewed was missing immunizations on file.”
“An electronic safety alarm on one bus has not been working since the start of summer. The operation uses this bus on field trips.”
“Loose fill under one playground structure does not meet at depth of at least six inches.”
“One caregiver was observed using their personal cell phone while children were napping.”
“Loose fill was not poured at an appropriate depth of six inches for a playground structure that is five feet tall or less.”
“This standard was tasked as part of an investigation and found deficient. An infant slept in a stroller for at least 10 minutes.”
“Caregivers attempt to keep infants awake if they appear sleepy outside of the scheduled napping hours. Caregivers have placed cold towels on the child's cheeks or walked them in a stroller to keep them awake.”
“This standard was tasked as part of an investigation and found deficient. Daily reports are not completed for the 12-17 month infant classroom.”
“Through interviews, it was determined that caregivers have been yelling at children.”
“Eight staff files reviewed and one director files reviewed did not have documentation for the required 24 and 30 hours of training as stated in standard 746.1309 and 746.1311.”
“Cleaning supplies were not marked. This was corrected during inspection.”
“Review of the medication authorization form. The operation did not document the following. 1. The dosage amount given 2. The date and time given This was corected at inspection.”
