Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One child admission form was observed missing the required health statement with the signature of the health care professional.”
“One 22mo child was observed sleeping on the couch during the inspection. This was corrected when the child was removed from the couch and placed on a nap mat during nap time.”
“It was noticed that two children s records did not contain Emergency Contact information. Note: The caregiver called the parents to obtain this information and placed it on the admission forms during the inspection.”
“One child's record did not contain designated contact information. Note: The caregiver called the parents to obtain this information and placed it on the admission forms during the inspection.”
“It was noticed that the fire extinguisher was not mounted on the wall. It was placed on the floor in the utility room. Picture was taken.”
“Three out of the eight children's records checked did not indicate if the child did or did not have any special needs. Note: The caregiver called the parents to obtain this information and placed it on the admission forms during the inspection.”
“The operational policy did not include the information of indoor and outdoor physical activity.”
