Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A 5 month old was sleeping in a play yard with toys hanging on the inside of the play yard. This was corrected when the primary caregiver removed the toys.”
“The primary caregiver and one household member did not have background checks submitted due to being inactivated by the primary caregiver on the provider portal.”
“The children's admission information from the four children files reviewed were missing; current immunization records, emergency contact address, and special needs limitations or restrictions.”
“The written feeding instructions had not been updated for a child not yet ready for table food, as well there were no written instructions to review for another child not yet ready for table food.”
“The primary caregiver completed 26 of the required 30 annual training hours.”
“Operation does not have liability insurance as of 5/24/23.”
“Provider is missing training hours from 2020 and 2021 and is currently working to get those hours completed to include hours for the new training year of 2022.”
“Observed four childrens records missing the special needs statement.”
