Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During the inspection 4 out of 6 staff files were missing the medication”
“An outdoor outlet did not have an enclosure and was accessible to children. This was corrected at inspection when safety covers were placed over the outlets.”
“A garbage can containing waste in the Pre-K 4 class did not have a lid making the material accessible to children.”
“Caregivers stated that cots were not being assigned.”
“The second director did not complete 30 hours training during the previous training year.”
“Three of ten staff files audited did not have an orientation statement on file.”
“A staff member was caring for children, unsupervised with standard provisional conditions in place. Note: This was corrected at inspection when caregiver was sent home.”
“Based on a DFPS investigation a two-year-old child was left in a full diaper. The caregiver responsible for the child at the time stated that the person who was responsible for the child before she took over the class should have changed the child, and it was not her role to change the child.”
“A caregiver grabbed a three year old child and pulled them to the wall.”
“A child did not have immunization records on file.”
“A child with a diagnosed food allergy did not have an allergy action plan in place.”
“Based on the evidence gathered it was determined that a caregiver yelled at the children in her care.”
“Based on the evidence gathered it was determined that a caregiver grabbed a child and forcefully placed them in a chair.”
“Three of ten employee files did not have a signed affidavit on file.”
“One of ten staff files did not have a copy of a high school diploma on file.”
“A staff member with access to children was present at the operation without background check status of Eligible, Eligible with Conditions, or Provisionally Eligible with Conditions. Note: The individual left the operation until cleared.”
“It was determined that supervision was compromised when a toddler exited the classroom without the caregivers' knowledge. The child was later found unharmed at the front office of the operation.”
