Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A caregiver was observed supervising infants prior to the operation receiving the results of the person's eligibility from CBCU.”
“Three of the classrooms in Building A, stored cleaning supplies, hand sanitizer and cleaning wipes within reach of children. It was discussed with caregivers and corrected at inspection. .”
“Three of the four vehicles where purchased after 2013 and did not have working alarms.”
“A seat belt in Van # 1 is cut, the front passenger seat is torn. Metal pieces and wire springs are visible and accessible. The front passsenger exterior door lock is broken. This vehicle is no longer going to be used as of 3-19-25.”
“A caregiver was observed to not change their gloves or wash their hands after diapering children.”
“A caregiver was observed to not sanitize the diaper-changing surface after each use.”
“The operation had a caregiver present around children without receiving cleared background check results.”
“A caregiver did not have a notorized affidavit available for review.”
“The woodchips on the playgrounds did not measure at least 6 inches in depth per standard.”
“A slate on the playgrounds fence was missing and needed to be repaired.”
“This standard was tasked as part of an investigation and was determined to be deficient. A caregiver did not use good judgement when allowing children to stand on and walk across chairs for an activity.”
“This standard was tasked as part of any investigation and was determined to be deficient. A caregiver did not used good judgement when they fed an infant applesauce that was not authorized to be fed to the child by the parents. This was corrected by the director speaking to the caregiver about the expectations. The director conferenced with the caregiver on 3/8/23 about the feeding expectations of the children in care.”
“This standard was tasked as part of an investigation and was determined to be deficient. A child in care that was not yet on table food did not have current feeding instructions available for review. This was corrected at inspection when it was determined that the child is no longer enrolled at the operation. Standard 746.2421(a) was discussed with the operation and the director and caregivers are aware of the need for written feeding instructions and how often they are to be updated.”
“The infant daily report was not current for a child that was in care. Operation did not update all feeding information, diapeer change information, sleeping information, mood, and summary of activities. This was corrected at inspection when the licensing representative discussed standard 746.2431 with director and ways that the caregivers can stay current on infants daily activites. Director stated that the operation will begin using paper forms to record infants daily activities.”
“Based on the investigation, a caregiver used inappropriate discipline.”
“A caregiver did not use self control with infants in care.”
