Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on the information obtained during the course of the investigation, caregiver grabbed and pulled a child by the arm.”
“Based on the information obtained during the course of the investigation, caregivers in charge of supervising the children failed to timely intervene when two children were pulling another child's pants down multiple times.”
“Nine employee files were reviewed. One employee completed the CPR training 100 % online. It did not include use of a CPR manikin or hands-on skill-based instruction.”
“Employee was observed not washing her hands and the children's hands after diaper changing.”
“Caregiver was observed separating two children, who were fighting, by moving a child to another area of the room. By only removing the child from the situation, caregiver did not use a discipline method that teaches the child acceptable behavior and self-control.”
“Caregiver was observed walking by two children engaged in an altercation and failed to intervene.”
“Ten children's files were reviewed. Two files were missing the immunization records. One file was missing an updated immunization record.”
“Diaper changing tables in two classrooms were observed to have dirt, and food crumbs under the changing mat.”
“Ten children's files were reviewed. Three files were pending updated immunization records.”
