Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Two children with diagnosed food allergies are missing completed food allergy emergency plans; both plans are missing the prescribed medications.”
“A child was left without caregiver supervision for 3 minutes in a classroom.”
“The operation did not keep documentation for 4 employees of training received for the following topics: Food Allergies, Administering Medication and Hazardous Materials.”
“The operation's fire inspection expired on 4/14/25.”
“A child was left unsupervised during a restroom break and walked into the adjoining empty classroom.”
“The operation is not completing a monthly inspection of the wall-mounted fire extinguishers. Note: this was corrected at inspection when the director performed a monthly check and annotated on the emergency practice drill log sheet.”
“1 out of 10 caregivers needs to complete pediatric CPR/ 1st Aid, not training for adults only and make the certificate available.”
“The operation is not completing a monthly inspection of the wall-mounted carbon monoxide detector. Note: this was corrected at inspection when the director performed a monthly check and annotated on the emergency practice drill log sheet.”
“The director's training certificate expired in January 2023.”
“The director's training certificate expired in January 2023.”
“Throughout the investigation and review of video it is evidence to support prohibited punishment”
