Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One of the classrooms had an outlet that was uncovered. This was corrected during the inspection when an outlet cover was placed in the outlet.”
“The parent handbook was missing an infant safe sleep policy, procedures for parents to communicate with the director, information on the abuse and neglect hotline and how parents can access minimum standards online, procedures for nursing mothers to nurse their children at the center, and information about the benefits of physical play and how children will do active play when the weather prohibits them from going outside. This was corrected during the inspection when this information was added to the handbook.”
“An outlet in a pre-k class was uncovered. This was corrected during the inspection when an outlet cover was placed in the outlet.”
“The first aid kit in the transportation bus was missing a guide to first aid, tweezers, and antiseptic wipes.”
“Out of ten children's files reviewed, three were missing hearing and vision screening results and two needed updated immunization records.”
“A caregiver was heard yelling at children during the inspection. This was corrected during the inspection when the caregiver was reminded that yelling is not an acceptable form of discipline.”
“During the DFPS investigation, there was sufficient evidence to show that a caregiver stepped into the bathroom to complete diaper changes, leaving the children in the classroom unsupervised.”
“During the investigation, there was sufficient evidence found to determine that adequate supervision was not provided when there were incidents where a child was bitten and the caregivers were unaware.”
“A classroom that had infants in it was using reduced naptime ratio. This was corrected during the inspection when another caregiver joined the classroom so that ratio was not reduced.”
“A caregiver who had conditions on her background check and couldn't be left alone with children was left alone with children during naptime. This was corrected during the inspection when another caregiver returned to the classroom.”
“There were two caregivers in classrooms who did not have a cleared background check yet. This was corrected during the inspection when the caregivers left the operation.”
“There was an infant asleep on a boppy pillow on the floor in the infant room. This was corrected during the inspection when the infant was moved to a crib.”
“In the afterschool classroom, there was a bottle of cleaning supplies left within reach of children. This was corrected during the inspection when the cleaning supplies were moved to a locked cabinet.”
“In one of the toddler classrooms, there was a bottle of multi-purpose cleaner sitting on a cabinet within the reach of children. This was corrected during the inspection when the cleaner was moved to a counter out of the children's reach.”
“There was an infant asleep on a boppy pillow on the floor in the infant room. This was corrected during the inspection when the infant was moved to a crib.”
“There was sufficient evidence found during the investigation to show that caregivers did not provide an adequate level of supervision to intervene and ensure the safety of children on the playground.”
“One infant was observed to be swaddled in a bare crib during the inspection. The swaddle was removed.”
“One classroom of 5 infants, ages 10 - 13 months, did not have their hands washed upon returning inside from outdoor play.”
“It was observed that the fence on the playground was not secured properly. Children were observed lifting the bottom of the chain link fence posing a safety risk.”
“There was sufficient evidence found during the investigation to show that the minimum director presence of 75% was not being met.”
